Hello, my name is Chloe. I have worked with Outcomes Connect Australia for the last year and a half as a Behaviour Support Practitioner. Previously, I was a part of a complex support worker team assisting individuals predominately with psychosocial and personality diagnoses and behaviour risks for 3 years. I pride myself on my education associated with vast topics of mental health and mental diagnoses/disabilities and how to best support individuals and their support network to better understand their current circumstances and to live with confidence.
